How to read unknowns and dates
“Unknown” means current cited evidence for that specific term is missing or out of date. It does not mean free, unavailable, or zero. A quote-needed care price is not a $0 price. When the underlying program review expires, that program leaves the current-program denominator until its source record is refreshed.
Medication charges can depend on the medicine, dose, pharmacy, coverage and eligibility. Ask for the full first charge, recurring care fee, medication amount, renewal date and cancellation method in writing. These published terms do not establish clinical safety or predict an individual bill.
Common questions
Does “monthly equivalent” mean I pay monthly?
No. For an annual-prepayment plan, the monthly figure is an arithmetic equivalent. Check the upfront amount and renewal terms in the cited source.
Does “medicine billed separately” tell me the medication price?
No. It only describes the billing boundary in the reviewed plan. The medicine amount may require a separate pharmacy or insurance quote.
Why can a cancellation rule be unknown when a provider has a cancellation page?
This table uses evidence attached to the exact program reviewed. If current dated evidence does not establish that plan’s cancellation terms, the audit leaves the field unknown.
